Who Is Eligible
Hospice care may be appropriate for individuals with a life-limiting illness whose physician believes life expectancy may be six months or less if the illness follows its expected course.

Meadowlark Hospice provides care in private residences, assisted living facilities, nursing homes, and other residential settings throughout our service area.
Family & Bereavement Support
Hospice care extends beyond the patient. Meadowlark Hospice provides emotional support, grief resources, and bereavement services for family members both during care and after the loss of a loved one.
Advance directives help ensure your healthcare wishes are understood and respected. We encourage patients and families to discuss advance care planning and complete the appropriate documents before they are needed.
