The Social Service/Discharge Planning Team help arrange for ongoing care during your stay and afterward. Our team evaluates your needs when you are first admitted to the hospital and works with you and your physician in acquiring home medical supplies and equipment, making arrangements for additional home assistance and alternative care planning. Our staff has put together a strong reference list of community services available to help you when you leave the hospital.
Referrals are available for specially trained professionals who provide a wide range of healthcare services in the home. Your physician will work with them to develop a personalized plan for intermittent or continuous care.
Your Social Service/Discharge
Planning Team will provide referrals to nursing homes and assisted-living
residences that provide care for those who need 24-hour medical or personal care. There are several such facilities in the community surrounding Clay County Medical Center.
