BENEFIT
ELIGIBILITY

MEDICAL INSURANCE
Employees are eligible for insurance the first of the month following employment. CCMC offers four levels of coverage (single, emp/spouse, emp/child, family), monthly rates vary accordingly. For the full-time employee, CCMC provides 70% of the premium cost and 50% of the premium cost for the part-time employee.

Preferred Health Systems provides medical and prescription drug insurance. Employees choose to take the insurance package or waiver out.

The plan is a $5000 deductible plan with a hospital paid Health Reimbursement Account (HRA) set up to help employees pay the high deductible. An employee pays a $1500 deductible. From $1501 to $5000 the dedctible is paid 50% by the employee & 50% by the hospital paid HRA.

All employees premiums are deducted from the employee's paychecks under the Flexible Compensation Plan. This means premiums are deducted from the employee's check on a pre-tax basis.

Full-time employees (60-80 hours/pp) and part-time employees (40-59 hours/pp) are eligible for insurance.

DENTAL INSURANCE
Delta Dental of Kansas are the providers for the CCMC group dental plan. Employees are eligible for insurance the first of the month following employment. CCMC offers four levels of coverage (single, emp/spuse, emp/child, family) with CCMC providing 49% of the premium cost for full-time and part-time employees.

Full-time employees (60-80 hours/pp) and part-time employees (40-59 hours/pp) are eligible for insurance.
DISABILITY INSURANCE
The hospital offers short-term and long-term disability insurance for all full-time regular employees. Premium costs are based upon employee's age and annual income. The premiums are payroll deduct.

LIFE INSURANCE
The hospital provides $10,000 of term life insurance and $10,000 of AD&D coverage for all full-time and part-time employees who work 40 or more hours per pay period. Additional supplemental coverage can be purchased for the employee and their dependents.

VARIOUS SPECIALTY INSURANCES
The hospital also offers a group Cancer, Accident, Heart/Stroke, and Vision insurances.

RETIREMENT PLAN
Mandatory– All eligible employees are required to paricipate in the plan. Any employee age 21 or older is eligible. This is a defined contribution plan with 3.5% of an employee's gross being withheld and deposited for them with American United Life. After an employee earns $15,000 gross in a calendar year the contribution level is raised to 7% for the rest of the year. These earnings are tax deferred under the 414(h)c IRS code. CCMC matches these contributions after one year of employment working 1,000 or more hours per year. Employees are 100% vested after eight years of employment working 1,000 or more hours in each of these years.

Voluntary- Both tax deferred and Roth after tax voluntary 403(b) annuities are available on a voluntary basis. These are not matched by the hospital.

TAX SHELTERED SPENDING ACCOUNTS 
Both medical and dependent care accounts can be established. These deductions are then run through the hospital's Cafeteria Plan and are deducted on a pre-tax basis.

HOLIDAYS  
Full-time employees have seven paid holidays per year. They are New Years Day, Memorial Day, July Fourth, Labor Day, Thanksgiving Day, Christmas Day, and one floating holiday. Other employees are paid up to 8hrs of double pay if they work the holiday.

VACATIONS  
Vacation leave is available to regular full-time, regular part-time and regular relief employees only. No vacation leave may be taken until after the first 6 months of employment. Vacation is accumulated on a prorated basis according to the number of hours worked in a pay period. An 80-hour per pay period employee would accumulate 80 hours of vacation the first year with one of those weeks available after 6 months of employment. After 5 years of employment, a regular full-time employee will receive three weeks or up to 120 hours of vacation per year. In the employee's 16th year, they receive 3.2 weeks – the 17th year 3.4 weeks are earned and so forth until they work up to four weeks of vacation their 20th year and each year thereafter.

ON-CALL PAY  
On-call pay will be paid at the rate of $1.25 per hour. If called back, the employee will be paid at time and a half for a minimum of one hour. OB call is paid at $3.00 per hour.

OVERTIME 
Only hourly employees are eligible for overtime pay, which is paid after 40 hours per week to regular employees. Special relief and PRN employees are paid overtime after 8 hours of work in one day.

SHIFT PREMIUM 
Shift premiums are paid to those employees working shifts other than the day shift. Base pay plus 7 percent is paid to those working shift 2. Those working shift 3 will be paid base pay plus 19 percent. Those working 12-hour shifts receive a prorated shift premium according to the above differentials.

BEREAVEMENT LEAVE
Full-time and part-time regular employees are granted paid leave for the death of an immediate family member. Employees are paid for any three days from the date of death to the day after the funeral services if needed and they are scheduled to work those days.

SICK LEAVE 
Sick leave is available to regular full-time and regular part-time employees only. Sick leave accumulates on a prorated basis according to hours worked in a pay period with an 80 hour employee accumulating one day or 8 hours per month. Sick leave begins accumulating the first day of employment but is not available for use until after 6 months of employment. Up to 32 hours per year can be used for the illness of an immediate family member.

RECOGNITION PROGRAMS  
Recognition is given to those employees who have given 5, 10, 15, 20, 25 and 30 years of unbroken service. Employees are presented with a gift catalog to select a gift as a reward for their years of unbroken service.

REDUCED MEALS  
A $20.00 value meal credit may be purchased for $10.00 from the Dietary Department. The cost of the meal will be deducted from the employee's next paycheck.

REST PERIODS AND MEAL BREAKS
The hospital provides a paid 15-minute rest period for each four hours worked. Employees are allowed a one half hour unpaid meal break.

CONTINUING EDUCATION
Continuing education opportunities are available to employees on-site, off-site and on-line through our web site - ccmcks.org

CCMC sponsored on-site and web based continuing education class offerings are free of charge to employees who work regularly. Discounts are available for PRN employees who don't work regularly.

TUITION ASSISTANCE
Full-time employees can receive up to $2,500/yr. in tuition assistance upon successful completion of approved courses with a grade of "C" (2.5 grade point) or better.

Part-time employees can receive up to $1,250/yr. in tuition assistance upon successful completion of approved courses with a grade of "C" (2.5 grade point) or better.

HOSPITALIZATION
If a private room is available, an employee requiring hospitalization will be given a private room at semiprivate rate.

Employees who work 40 hours or more per pay period will receive a ten (10%) percent discount on all hospital services. This discount applies to the portion of the bill after insurance. An additional five (5%) percent on the after insurance portion will be allowed if the bill is paid in full within 30 days following a mailer statement from the hospital after payment by the insurance company. Should an employee not have any insurance coverage, the discount will apply to the total bill. There is a $1,500.00 maximum family discount per year.

Free membership for employee and immediate family members are provided to the Mark A. Chapman Wellness Center.
